Hi there,
Thank you. Yes those 3 checks that say Corewell on that date were also done at the Starfish event. Corewell did support the event too though. Since the State seats were provided via the Starfish inventory, maybe it makes sense to switch them over to Starfish?

The two ID below look like they were created by a Julie Kaye and tagged Corewell Health instead of Starfish. Should they be moved over to Starfish?
See attached for an export of the Michigan checks on that date that look like they could it. Let me know if any of those need to be changed.
It may be an instance where the check is still stored on the iPad and hasn’t been submitted into the system. If so, I can’t see it until it gets an ID#.

I hope all is well. Thank you for assisting Carolyn and me regarding the NDCF check form assignments and the agency information listed within the database.
Today, Carolyn and I noticed that we have three files that are not displaying in the system with an NDCF Form ID. One file is from May 8, 2026. The parent completed and signed both the electronic and physical form, and they received two donated car seats during their individual training and fitting appointment. Although the file was saved, we are unable to locate or view it within the system.
The additional two files are from the May 1, 2026, Starfish Family Services Car Seat Event,
The file numbers are listed below:
• NDCF File #: 1868976
• NDCF File #: 1868856
• Starfish Family Services Car Seat Safety Check Event form May 1, 2026, data displays 12 forms, 20 children but we have 14 forms and 22 children serviced the day of the event.
May you please assist us in locating these files and sharing with us the best practice and procedure when this happens? We truly appreciate your support and assistance.

A couple of us at Starfish Family Services are completing the process to request to be program managers for the account on carseatcheckform.org.
As far as we know, there is no current program manager assigned to the page.
I just had a question regarding the request form. There is an area where it asks for an "Agency Authorized Representative" to sign off on the form. Is this just our direct supervisors, even if their names are not otherwise on the car seat check form webpage? This is to make them aware so that they can make any changes if staff move on from the agency, correct?
We each have different supervisors, so these would be different names signing our request forms.
