Hello,

Please delete form #1840476.
The CPST working on the call, Elena Kutchey, input the information provided by the caregiver (vehicle and zip code, etc.) to create a form and form number prior to actually interacting with the family.

The video call was not completed due to a language barrier, so it was not possible to conduct an inspection of the car seat or installation.
